We have now made it easier for you to save time during your first visit by filling out all the necessary forms on your computer or mobile device. We ask all of our new patients who have not yet filled out this form to do so to assist us in advancing your care.
We value your time. You will be pleased to find that we will generally see you right at the appointed time. We will make every effort to complete your treatment in as few visits as possible. In order for us to be efficient with the time that we have scheduled for you, we would ask that you also be prompt.Your dental health is important. Your appointment is an important part of maintaining your dental health. Keeping these goals in mind, it is our policy to charge for missed appointments, unless cancelled, (Monday through Friday) during normal business hours, at least 2 business days in advance.
We accept most insurance plans and will gladly process your claim. Please bring the necessary information with you on your first visit.Insurance policies generally cover only a portion of the total treatment cost. Unless other arrangements have been made, we ask that you pay your portion of the bill at the time of treatment. It is your responsibility to pay any balance not paid by your insurance company.For your convenience, we also accept payments made with your Visa, MasterCard, and Discover. Financing is available through CareCredit available at our office.If you have any questions on insurance and billing, please call our office at 413-426-9632 to ask about insurance and billing.
We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see or call our Financial Coordinator for details. We are happy to file insurance for your reimbursement as long as you are free to choose your own dentist.
